biihead – Underlined page headings

De­fines a \pagestyle{bi­ihead­ings} in which the head­ing is un­der­lined, and con­tains chap­ter and sec­tion num­bers and ti­tles. (Thus, of course, it only works in re­port and book classes.
The pack­age was writ­ten for LaTeX 2.09, pre­sum­ably be­fore the fan­cy­head­ings fam­ily had be­come avail­able.
